Chlamydia Screening

Are you aged 15-24?  If yes, please contact the surgery and ask to make an appointment for your free chlamydia test.  It only takes a few minutes to do and results are sent to you confidentially.

Cervical Screening Programme

We recommend that all women aged between 25 and 64 regularly attend for cervical screening.  Cervical screening has reduced cancer rates by 50% since it first began.  You will be written to inviting you to attend either every 3 years or 5 years dependent on your age.  When you receive your letter, please telephone the surgery to make an appointment with the practice nurse.

Pre-Registration with the practice

When you have completed all of the details, click on the "Send" button to mail your form to us.   Please note that you will be contacted by a Receptionist to confirm if we are able to register you.  We are currently unable to register patients living in the Cedars Park or Chilton Hall areas of Stowmarket.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
