NHS Health Checks

The surgery is pleased to announce that patients will be able to attend for an NHS Health Check from November 2011. These checks are for patients aged 40-74 who are not currently being seen regularly by their Doctor. If you are eligible you will receive a letter inviting you to make an appointment.

Chlamydia Screening

Are you aged between 15 and 24? Have you had a free chlamydia screening test? You can have the test here at the surgery and the service is completely confidential. Just ask for an appointment with a practice nurse.

Update on measles outbreak declared on Merseyside

A measles outbreak has been declared on Merseyside. There are 13 confirmed and 16 suspected cases ranging in age from nine months to adults aged in their 20s. Six people, four children and two adults, required treatment in hospital. This compares to just one confirmed case on Merseyside in the first six weeks of 2011 and one in the corresponding period of 2010.

Update on Legionnaires’ Cluster Associated with a Hotel in Spain

10 February 2012, 1400hrs. The Health Protection Agency (HPA) has this week been alerted by the Spanish public health authorities about a further case of Legionnaires’ Disease in a UK resident who stayed at the Diamante Beach Hotel in Calpe, Costa Blanca, as well as two French cases. This brings the total number of UK residents associated with this cluster to 12, with three deaths.

The population of Stowmarket continues to grow and with the agreement of NHS Suffolk, Combs Ford Surgery’s practice area is as defined by the map below, with the exclusion of the Cedar’s Park and Chilton Hall housing developments.

If you live in our practice area and would like to register with us, please complete one of our registration forms that are available from Reception. Our staff will then inform you which of our team of doctors is taking on new patients. Please note that Dr Caroline Dunmore and Dr LP Koratamaddi do not have their own list.

PreRegistration If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. Please note that you will be contacted by a Receptionist to confirm if we are able to register you. We are currently unable to register patients living in the Cedars Park or Chilton Hall areas of Stowmarket.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
